List of California Districts Missing Item 21 Attachements - May 15th, 2013 Deadline (Excel, 3 MB)
This Excel document contains a list of all the FY2013 Funding Request Numbers (FRNs) in the USAC database that we believe are missing Item 21 attachments. You MUST submit an Item 21 attachment for each of your FRNs on or before May 15, 2013. FRNs that are missing Item 21 attachments or have Item 21 attachments submitted after May 15 will be denied for being out-of-window.
