USAC Fall E-rate Training
On Friday, September 7, the Universal Service Administrative Company (USAC) announced their fall 2018 E-rate in-person training schedule. We are happy to announce that one of the four regional trainings will be held in Sacramento.
There are only 54 seats left in this training. We encourage interested California E-rate applicants to register as quickly as possible.
The Sacramento dates and times are listed below: (REGISTER)
- Thu Nov 1, 2018: 12:30 – 5:00 PM (Optional beginner session – Capacity 90)
- Fri Nov 2, 2018: 9:00 AM – 4:30 PM (Capacity 280)

Pending Dates and Deadlines
E-rate deadlines may be viewed or calculated on the Universal Service Administrative Company (USAC) website at https://www.usac.org/sl/tools/deadlines/default.aspx
September 30, 2018 Service Delivery Deadline
The deadline for delivery and installation of non-recurring services such as Category One broadband installation and Category Two Internal Connections is generally September 30 following the close of the funding year. Applicants with funding requests that contain non-recurring products or services that have not yet been delivered or installed must start working now on filing the FCC Form 500 to extend the service delivery dates.
The Service Delivery and Contract Extension Request must be submitted to USAC on or before the September 30, 2018 deadline. Requests received after this deadline cannot be processed by USAC. The applicant must request a service delivery extension by filing the request on an FCC Form 500. Requests that come directly from a service provider cannot be processed.
September 30, 2018 Service Substitution Deadline
The deadline, to file a service substitution is on or before the last day to receive services for the applicable funding request, is:
- June 30 of the relevant funding year for recurring services, or for Category One special construction charges
- September 30 that follows the close of the funding year for non-recurring services Category One installation and Category Two internal connections. For Funding Year 2017, the deadline for non-recurring services is September 30, 2018.
FCC Form 486 Filing Reminder
The FCC Form 486 (Receipt of Service Confirmation and Children's Internet Protection Act Certification Formnotifies USAC that the billed entity and/or the eligible entities that it represents is receiving, or has received, service in the relevant funding year from the named service provider(s).
Funding Year 2018
The First FCC Form 486 deadline for FY 2018 will be October 28, 2018.
Applicants missing these or earlier deadlines should closely monitor their News Feed in the E-rate Productivity Center for an FCC Form 486 Urgent Reminder Letter notification. This letter will allow the applicant a 15-day extension to submit an FCC Form 486 without penalty.

E-rate Process: Activities to Work on Now
FCC Form 498 Service Provider and Billed Entity Identification Number and Contact Information Form
Starting in Funding Year 2016, USAC required that applicants receive E-rate reimbursements electronically via Automatic Clearing House (ACH) transfer. Applicants must complete the FCC Form 498, Service Provider and Billed Entity Identification Number and Contact Information Form, and provide banking information in order for USAC to electronically transmit funds to their accounts. The FCC Form 498 must be completed and approved before the applicant can receive any FCC Form 472 BEAR reimbursements.

If your entity does not have an approved FCC Form 498, or if the current FCC Form 498 needs to be amended, consult with USAC’s User Guide for step-by-step instructions: https://www.usac.org/_res/documents/li/pdf/forms/FCC-Form-498-Applicant-User-Guide.pdf.
Gathering Required Information
The following information will need to be collected in order to complete FCC Form 498:
- District’s Federal Employer ID Number (FEIN) or Taxpayer Identification Number (TIN)
- Dunn and Bradstreet Identification Number (DUNS). If you do not know your DUNS number you can contact Dunn and Bradstreet, Inc. at 1-866-705-5711
- Remittance Contact
- Remittance Financial Institution
- Financial Institution Account Number for ACH
- ACH Financial Institution Transit Number
- First page of bank statement
The information listed above is required to complete the form as well as to respond to USAC’s request for information once the form has been submitted. USAC will contact the person who certified the form to request documentation that verifies the bank account owner, the routing number, and the account number. This documentation may be a canceled check or bank statement. USAC cannot complete the processing of the FCC Form 498 without this information.
Requesting a PIN for the FCC Form 472 Billed Entity Applicant Reimbursement Form (BEAR)
The FCC Form 472 (BEAR) can only be filed online through the Schools and Libraries Division legacy system. In order to submit the BEAR, the person completing the form must obtain a Personal Identification Number (PIN). A PIN is requested by creating a Customer Service Case through EPC or contacting USAC’s Client Service Bureau (CSB) at (888) 203-8100 and submitting a completed “PIN Request Template” spreadsheet that can be found on USAC’s website: https://www.usac.org/sl/tools/forms/pin.aspx.
USAC will send an email with the assigned Case Number to the EPC user that submitted the Customer Service Case which will contain a link to the Customer Service Case in EPC. Once the PIN has been created, a USAC Notification will be issued indicating that the Case Number has been modified. USAC will notify the EPC user that a PIN has been created and will be emailed to within in a certain timeframe. As always, it’s best to be proactive by periodically checking on the status of your Customer Service Cases in EPC.
